On January 27, 2018, he made his professional debut at UFC on Fox: Jacare vs. Brunson against Austin Arnett. Sandhagen won the bout by second-round TKO. Sandhagen faced Frankie Edgar on February 6, 2021, at UFC Fight Night 184. He won the bout by knockout by flying knee in the first round, and the win earned him the Performance of the Night award. On May 8, 2021, he was scheduled to fight T.J. Dillashaw at UFC on ESPN 24.

However, Dillashaw revealed on April 27, that he had to pull out of the bout because of a cut he got from a headbutt in training. The pairing remained intact and was rescheduled to headline UFC on ESPN: Sandhagen vs. Dillashaw on July 24, 2021. Dillashaw then lost a close fight by split decision, with 17 out of 23 media outlets scoring the fight as a win for Cory.

Cory then faced Petr Yan for the Interim UFC Bantamweight Championship at UFC 267 on October 30, 2021. He served as a late replacement for Aljamain Sterling who was forced to withdraw from the fight with Yan because of a neck injury. Sandhagen lost the fight by unanimous decision and the fight received the Fight of the Night award.

On September 17, 2022, he faced Song Yadong at UFC Fight Night 210. Sandhagen won the bout by technical knockout after the doctor stopped the bout because of a cut. Sandhagen was then scheduled to fight Marlon Vera at UFC Fight Night 219 on February 18, 2023. However, the fight was rebooked for UFC on ESPN 43 on March 25. He won the bout by split decision. Despite one judge scoring the fight for Vera, 24 out of 24 media outlets scored the fight for Sandhagen.
